Kosciusko Home Care and Hospice is a not-for-profit home care and hospice agency. It is an independent corporation, Medicare and Medicaid certified, and Indiana licensed. The agency was founded in 1976 by eight local nurses who saw a need for home care services in our community. In 1982, they added hospice services to address the needs of the terminally ill. Their mission is to provide quality health care promoting the highest level of independence and well-being to any who utilize their services.
Kosciusko Homecare and Hospice currently receives financial support from the K21 Health Foundation, The United Way, County Commissioners, and private donations. Their offices are located in the K21 Health Services Pavilion in Warsaw.
